EDCC Program Information
The following registration forms can be completed interactively on your computer within the template darkened areas (you can tab from field to field) or you may print the blank forms to complete by hand. The registration forms must be completed in one session; some users may find that the file cannot be saved for later access. Also with some browsers you may have to download the forms to your computer for full functionality.
Click on the appropriate link below and allow at least one minute for the pages to download. There are 7 pages in each registration packet. When completed, please print; you must initial and sign where needed, attach your registration fee and deliver the completed forms to the Extended Day Care Supervisor at your determined school site.
