Club APPLICATION FORM

**ALL clubs will submit new/revised applications for the 2012-2013 school year.

Club CONSTITUTION OUTLINE

*ALL clubs must have an approved constitution on file as of December, 2012.

**Must submit a typed constitution when apply for club approval.

FUNDRAISER REQUEST FORM:

All on-campus club fundraisers MUST be pre-approved as of December 1, 2011. This form, in addition to a scheduled meeting with the Activities Director, must take place AT LEAST 2 weeks before your club fundraising activity is to begin. No exceptions.
