2022-2023 Extended Day Care Enrollment Information and Requirements
· 2022-2023 Emergency and Student Information Sheet
Families are asked to complete a 2022-2023 Emergency and Student Information Sheet, for each student, initial/respond to the items at the bottom of the sheet, sign, and deliver your completed form along with payment covering the registration fee to your school’s Extended Day Care Center during program hours, Monday through Friday, 6:30 AM to 6 PM.
· Payment covering the $90 registration fee, per student, is required to process your enrollment request.
Cash or checks made payable to: Los Alamitos USD and should include your student’s name in the memo. Families with multiple students may submit one check covering the registration fees. To avoid confusion, we kindly ask families to submit their student’s enrollment form and payment together to their school’s Extended Day Care Center (emailed forms without payment do not guarantee enrollment and will not be processed).
· Online Registration/Re-Enrollment/Parent Portal Account Required
Once your student’s enrollment paperwork and payment are received and processed, an email will be sent to you that will include a link and instructions on how to register online and how to create a parent portal account in our new system, DayCare Works, if necessary. Please note: Online re-enrollment must be completed prior to your student's requested start date.
Online re-enrollment requests will be processed at the District Office within 1-2 school days.
Monthly Calendar Request Forms can be picked up/completed at your school's Extended Day Care Center.
